The Brazilian prison system is experiencing a significant increase in its incarcerated population, which poses challenges to ensuring the right to health. In this context, physical therapy can contribute to health promotion and the prevention of health issues. This study aims to report on the experience of physical therapy practice within the penitentiary system, with an emphasis on health promotion initiatives. It is an experience report developed at the Doutor Mário Negócio State Agricultural Penal Complex in Mossoró, Rio Grande do Norte (RN), between May 2024 and May 2025, as part of a Multiprofessional Residency program in Primary Care, Family Health, and Community Health. Mat-based Pilates sessions were conducted with inmates in the resocialization unit, focusing on stretching and muscle strengthening. Participants showed good adherence to the program, reporting a reduction in lower back pain and spontaneously practicing the exercises within their cells. It is concluded that simple, low-cost physical therapy interventions can contribute to health promotion, improved functionality, and the encouragement of self-care within the prison environment.Downloads
